On May 13, 2001, Miller died of a heart attack in his hometown of Scranton, Pennsylvania. In 2004, actor Paul Sorvino, a longtime friend of Miller and a cast member of all three versions of That Championship Season, was commissioned by Scranton to create a bronze bust of the late playwright and actor. The statue was unveiled in December 2008.

He was born in Long Island City, New York, but his parents moved to Scranton, Pennsylvania, when he was an infant. Children: Jennifer Miller, Jason Patric, Jordan Miller, Joshua John Miller. Parents: John and Mary Collins Miller.

As a result of the affair with Delgado, Miller became the father to a baby boy in July 2017. In January 2017, Miller sold his interest in Jamestown Associates, and joined Teneo Strategy.
